Please complete the membership application form by downloading the PDF (follow the link button below) and complete using one of the following options:
1. To complete electronically
Download and enter the information into the form electronically, then use the SUBMIT button to send.
2. To complete manually
Download and print the form, fill it in, scan and email it to membership@hae.org.uk.

All applications are subject to a 2-week cooling off period. During this time the Membership Team will be in contact to create your full profile and put together your Members Pack.
